All Credentialed Doctors

Medical Staff Organization

Makati Life Medical Center

Dear Colleagues:

Subject: Invitation to be Accredited with Partner Payors

From our 24/7 Outpatient Department to the full operation of our Level 2 hospital, Makati Life’s journey, now marked by PhilHealth accreditation, demonstrates that Public-Private Partnership can truly deliver people-centered, technology-enabled care. But beyond the infrastructure and milestones, what truly sets us apart is a shared commitment to purpose, which is a deep sense of social responsibility, and a collective drive to make healthcare more accessible, equitable, and compassionate for all.

In this spirit, we invite all credentialed doctors to take part in our blanket accreditation initiative with Partner Payors that broaden access to care for the communities we serve. These include:

  • Makati Health Plus Program (Yellow Card) of the City Government of Makati
  • Medical Assistance for Indigent and Financially Incapacitated Patients (MAIFIP) of the Department of Health
  • Local Health Maintenance Organizations (HMOs), Insurances, Brokers, and Third-Party Administrators (TPAs)

The details of the accreditation program are as follows:

  1. For HMOs
    • Professional fees will follow the compensation schemes agreed upon by your respective societies with AHMOPI (Association of Health Maintenance Organizations of the Philippines, Inc.) and PAHMOC (Philippine Association of Health Maintenance Organization Companies), applicable to both outpatient and inpatient services.
    • All other applicable rules and guidelines under standard private practice agreements with HMOs shall likewise apply.
  2. For the Makati Health Plus (Yellow Card) Program
    • Standard HMO-based compensation rate formula, less 7% administrative fee* for hospital processing apply.
    • Active consultants are automatically considered part of the Yellow Card network. Their existing privilege status implies consent to the above compensation scheme.
  3. For Medical Assistance for Indigent and Financially Incapacitated Patients (MAIFIP)
    • Professional fees must not exceed 50% of the approved amount of assistance, less 7% administrative fee* for hospital processing.

      * For government-sponsored programs such as the Makati Health Plus (Yellow Card) and Medical Assistance for Indigent and Financially Incapacitated Patients (MAIFIP), a 7% administrative fee will be deducted by the hospital for processing.

Please note that certain Partner Payors may require supplemental documentation beyond our standard credentialing process. This includes annual renewal of your participation, as necessitated by evolving policies and system updates from our partner institutions. Relevant data already on file with our credentialing unit will be shared accordingly, and your conforme to this letter will serve as your consent to release such information.

Enclosed is the Conforme Form for you to indicate your participation. Kindly return the signed form on or before July 15, 2025, either by submitting it to the Medical Director’s Office, 5th Floor, Makati Life, or via email at mdaccreditation@makatilife.com. If we do not receive a response by the deadline, we will consider your continued active privilege as implied consent to participate in the accreditation program.
